Left opponent can only use sabers / melee combat, but can of course still use the Force for augmentation / precog / etc.
Right opponent gets no weapon

Starting distance: 20 meters

Mace Windu (peak vaapad) vs. RotS Sidious - Mace is already a peer of Sheev's and was doing pretty well against Sidious' lightning while exhausted. A fully rested Mace with his lightsaber should be able to take a very slight majority, though Vaapad is virtually useless here.
Yoda vs. RotS Sidious - Not sure, tbh. 
RotS Sidious vs. RotJ Sidious - Not sure how big the gap is between them but it shouldn't be enough for ROTJ Sheev to stomp.
IH Obi Wan vs. Dooku - Dooku has already shown that he can ragdoll Obi-Wan if given even a slight opening.
Kit Fisto vs. IH Obi Wan - Gap isn't too big.
Revan vs. Revan - Um, which Revan? 
Caedus vs. LotF Luke - No idea.
Ventress vs. Maul - Gap between them isn't too big for Maul to dominate with the Force.
Outlander vs. Vaylin - Outlander is already more powerful if end of KOTET.

Yoda vs. RotS Sidious - Hmmm. I'd favour Yoda, but I'm unsure.

RotS Sidious vs. RotJ Sidious - ROTJ Sidious.

IH Obi Wan vs. Dooku - Dooku has repeatedly shown he can dispatch of Kenobi with TK. There's no reason why he shouldn't win.

Kit Fisto vs. IH Obi Wan - I feel like Fisto would probably be written to win this fight, but by feats Kenobi takes it.

Revan vs. Revan - I'll back Saber Revan, though I could see a case for Force Revan.

Caedus vs. LotF Luke - Luke ragdolls.

Ventress vs. Maul - Ventress.

Outlander vs. Vaylin - Outlander.

Malak vs. KotOR Revan - Unsure, it's not clear how close they are in Force Power.

Maul's offensive Force capabilities are very limited and he isn't above Ventress in such a way that he can outright ragdoll her.

Why do you have Ventress winning against Maul?

I don't honestly see why Maul should win. There isn't any massive gap in Force Power imo, so Ventress should be able to get in CQC with him and win.
